Either this guy has eaten the muzzle of a Tokarev, has fallen down the stairs, or has gotten radiation poisoning from polonium.
One other scenario...they have absolutely no clue who did this but apparently it is causing problems in Russia so they need everyone to forget about it. As well, maybe they think the US penetration will get lax if he/she is still in place.
